Our clinic is located at 640 Eisenhower Blvd. Suite 200.  We are inside the Milestone Square building. I am in the clinic 3 days a week, Monday - Wednesday.  I am also in clinic every other Friday.  

Our office is upstairs. Enter through the front door facing Hwy 34 and come up the stairs to your right.
